Spontaneous middle cerebral arterial dissection presented with limb shaking.

چکیده
Middle cerebral artery dissection (MCAD) is a rare cause for ischemic stroke and is often associated with headache and hemispheric symptoms. In the present report, limb shaking is found to be a major symptom of spontaneous MCAD in a young male. The diagnosis of MCAD is based on the intramural hematoma directly visualized in MR T2-weighted images and supported by serial evolution of transcranial doppler sonography. This presentation suggests that limb shaking can be a symptom of severe intracranial carotid occlusive disease.

Shadow sign in a T2* brain image in spontaneous middle cerebral artery dissection.
A29-year-old man presented with unilateral dull headache for 3 days, similar to previous headache, but with right hemiparesis; there was no antecedent infection or trauma. MRI showed left striatocapsular infarction (figure 1A), without evidence of cardioembolism or vasculitis. Digital subtraction angiography demonstrated stenosis of the left middle cerebral artery (MCA) trunk (figure 1B). Intra...
متن کاملPostcoital carotid artery dissection associated with acute cerebral infarction: a case report.
Carotid arterial dissections may result from spontaneous or traumatic causes. Postcoital arterial dissections have been reported in both the vertebral and coronary arteries. We report a rare case of spontaneous dissection from the common carotid artery to the internal and external carotid arteries after sexual intercourse, leading to fulminant middle cerebral artery (MCA) territory infarct. Alt...

RATIONALE Intracranial arterial dissection is a rare cause of ischemic stroke, and isolated middle cerebral artery dissection (MCAD) is extremely rare, having been described only in sparse case reports. The etiology, clinicoradiological features, and treatment strategies are not yet well understood. PATIENT CONCERNS A 49-year-old man presented with rapidly progressive aphasia and motor distur...
متن کاملLimb-shaking transient ischemic attack responsive to nimodipine: A case report
Limb-shaking transient ischemic attack (TIA), a rare manifestation, is commonly caused by severe stenosis or occlusion of an extracranial internal carotid artery. Such patients are usually treated with surgical revascularization or anti-platelet therapy. We present a 56-year-old woman with 6 months’ episodic attacks starting with mouth skewed to the right and a sensation of ‘weakness’ involving...

BACKGROUND Spontaneous intracranial arterial dissection (IAD) is an increasingly important cause of stroke, such as subarachnoid hemorrhage (SAH) and hemodynamic or thromboembolic cerebral ischemia. IAD usually occurs in the posterior circulation, and is relatively rare in the anterior circulation including the middle cerebral artery (MCA). Various surgical and endovascular methods to reduce bl...
